These lists include medals won at the 1906 "intercalated" Olympics in Athens, which are not recognized by the International Olympic Committee. In the early days of the modern Olympics, teams didn't necessarily represent a single country and occasionally an individual athlete represented more than one country. When a team of mixed nationality won a medal, it's credited jointly to the countries. Similarly, if an athlete was considered to represent more than one country, that athlete's medal is credited jointly to the countries involved.
